What is NAD+ and why does it matter?

Nicotinamide adenine dinucleotide (NAD+) is a coenzyme present in every living cell. It plays a central role in energy metabolism, DNA repair, and cellular signaling. NAD+ levels decline with age: by age 50, most people have roughly half the NAD+ they had at 20.

This decline is not just a biomarker. It appears to be mechanistically linked to multiple hallmarks of aging, including mitochondrial dysfunction, genomic instability, and inflammatory signaling. The question is whether restoring NAD+ levels can reverse or slow these processes.

The precursor landscape: NMN vs NR vs NA

Three main precursors can boost NAD+ levels:

  • Nicotinamide mononucleotide (NMN): Converts to NAD+ via the salvage pathway. The most studied precursor in recent years, particularly after David Sinclair’s research brought it mainstream attention.
  • Nicotinamide riboside (NR): Marketed as Niagen, NR was the first NAD+ precursor to gain significant commercial traction. Converts to NMN first, then to NAD+.
  • Nicotinic acid (NA/niacin): The oldest known NAD+ booster. Effective but causes flushing at therapeutic doses, limiting compliance.

What the clinical trials actually show

The human evidence base is growing but still limited. Most published trials are small (under 100 participants), short-term (8 to 12 weeks), and measure NAD+ levels in blood rather than functional outcomes like healthspan or disease prevention.

NMN and NR supplements range from $30 to $120 per month depending on brand, purity, and dose. Given the current evidence base, this is a personal risk-tolerance decision rather than a clear medical recommendation.

Generic niacin (nicotinic acid) costs under $5/month and reliably raises NAD+, but the flushing side effect makes it impractical for many people. Extended-release niacin reduces flushing but carries liver toxicity concerns at high doses.

Bottom line

NAD+ decline is real, measurable, and mechanistically linked to aging. Precursor supplementation reliably raises NAD+ levels. The gap is between “higher NAD+ levels” and “better health outcomes,” and that gap has not yet been closed by human clinical data. The research is promising but incomplete.
